For a several-month gap, treat as a complete fresh start — clinical assessment, starter dose, full titration, the lot.

Restarting Wegovy after a break

For a 2–4 week gap on Wegovy:

  • From 0.5mg or 1mg: drop back to 0.25mg for 4 weeks, then re-titrate
  • From 1.7mg: drop back to 1mg for 4 weeks
  • From 2.4mg: drop back to 1.7mg for 4 weeks, then return to 2.4mg

For a 4+ week gap, restart at 0.25mg and re-titrate through the full 16-week schedule. This is the licensed default.

Why the cautious restart matters

A common pattern: someone stops Mounjaro 12.5mg for 6 weeks, then thinks "I'll just resume where I was". They take a 12.5mg dose. The next 24–48 hours involve severe nausea, vomiting, possible dehydration, and a return of any side effects from earlier in titration — but worse.

By contrast: dropping back to 5mg or 2.5mg for the restart, then stepping up over 8–12 weeks, replicates the gentle on-ramp that worked the first time. Side effects are mild, the medicine resumes its effect, and the patient gets back to their previous maintenance dose without crisis.

Supply issues
