Switching to Retatrutide

You can't legally switch to retatrutide in the UK right now

Switching from a licensed medicine to an unlicensed one through a regulated UK provider is not possible. No GPhC-registered UK pharmacy can supply retatrutide because the MHRA has not granted marketing authorisation for it. Any prescriber who attempted to do so would be operating outside the regulated supply chain.

This is the same status as for any unapproved prescription medicine in the UK — not a temporary issue, but the ordinary process of medicine approval working as designed.

Why "switching" via unregulated sources is a particularly bad idea

Some patients on Mounjaro or Wegovy are tempted by unregulated retatrutide sellers offering what they claim is the "newer, better" option. From a clinical standpoint, this is the worst possible switching scenario:

You lose the regulated supply chain protections you currently have

If you're on licensed Mounjaro or Wegovy through a regulated UK pharmacy, you have:

  • Verified, manufacturer-supplied product
  • A clinical assessment that screened your contraindications
  • A prescriber to call if anything goes wrong
  • Established dosing and titration

Switching to unregulated retatrutide drops all of these. You would be giving up real safety infrastructure for a vial of unknown provenance.

You introduce overlapping medicine risks

If you stop Mounjaro or Wegovy abruptly to start unregulated retatrutide, you bypass the standard washout that minimises overlapping receptor activity. With unregulated product, dose accuracy is unknown, so the overlap risk is harder to estimate.

Comparing the wrong things

The phase 2 trial figure of around 24% average weight loss for retatrutide at 12mg is for trial-grade product administered under trial conditions. It does not apply to what is in an unregulated vial. The "extra weight loss" patients hope to gain from switching may not actually exist if the unregulated product is incorrectly dosed or contaminated.

What to do instead

If you are unhappy with your progress on Mounjaro or Wegovy, the legitimate options are:

1. Optimise your current treatment first

Many "I want to switch" requests turn out to be addressable without switching:

  • Are you on the maximum tolerable dose? Many patients on Wegovy have not reached 2.4mg, or on Mounjaro have not reached 10mg or 15mg. Higher doses often unlock further progress.
  • Are you adhering to the schedule? Weekly injections, on the same day, with no missed doses, give the best chance of progress.
  • Are diet and activity in line with the medicine's effect? The medicines reduce appetite but do not override active overconsumption.
  • Are there other contributing factors? Other medicines that promote weight gain, untreated thyroid issues, sleep apnoea, mental health, alcohol intake — all worth a review.

2. Switch between Mounjaro and Wegovy if appropriate

3. Take part in a retatrutide clinical trial

If retatrutide specifically is what you want, the only legitimate way to access it in the UK at this time is through a registered clinical trial. Trial participation is free and includes:

  • Detailed clinical assessment
  • Trial-grade product manufactured by Eli Lilly
  • Safety monitoring with blood tests, ECGs, and clinical review
  • Follow-up care

Note that trial enrolment depends on the trial's specific eligibility criteria, which vary. Many trials cap enrolment quickly.

4. Wait for MHRA approval

If retatrutide is approved in due course, switching from Mounjaro or Wegovy to retatrutide will become a routine clinical option through regulated UK pharmacies. There is no published date for approval, so the wait could be measured in years.

What a future legitimate switch might look like

When retatrutide is licensed in the UK, the standard switching approach (drawing from how switches work between currently licensed medicines) will likely involve:

  • Stop the current medicine completely
  • Wait a washout period specific to the medicine being stopped (likely 4–6 weeks for the medicine being stopped)
  • Start retatrutide at its starter dose, regardless of the dose reached on the previous medicine
  • Follow the standard retatrutide titration

This is forward-looking expectation, not regulatory guidance. The actual switching protocol will depend on the licensed retatrutide schedule, which doesn't yet exist.

What to tell your prescriber

If you've already obtained retatrutide from an unregulated source — or are considering doing so — talking to your current prescriber matters. Specifically:

  • Disclose what you've taken or plan to take. Concealing it makes your medical record incomplete and risks worse advice if something goes wrong.
  • Discuss the safety risks honestly. Your prescriber may have specific concerns based on your medical history.
  • If you've already used unregulated retatrutide and had side effects, report them through the MHRA Yellow Card scheme. Even unregulated product side effects help the regulator understand what's circulating.

A prescriber who learns mid-treatment that you've been using unregulated supply may decline to continue prescribing. That is a clinical judgement call, not a punishment.

Frequently asked questions

My friend switched to retatrutide and lost a lot of weight. Is it really that risky?
Possibly your friend is using genuine product and has been lucky. Possibly they've not had any of the contraindications. Possibly they don't know they're using a counterfeit and are losing weight for unrelated reasons (e.g. severe nausea from contaminants). Anecdotes from friends are not a reliable basis for switching from a regulated to an unregulated supply.

Can my private prescriber order retatrutide for me from another country?
No. Importing unlicensed prescription medicines into the UK without specific MHRA clearance is unlawful. Regulated UK prescribers will not do this.

What if retatrutide gets approved in the US first — can I import it from there?
No. UK regulations on prescription medicines apply regardless of approval status elsewhere. Customs may seize parcels of imported prescription medicines.

How will I know when I can legitimately switch?
